In Year 9, the Annual Review is called a 14+ Transition Review. It is at this time that a Transition Plan is drawn up to help plan for your child’s future.
Key Information
At the review, an Additional Needs Adviser (ANA) or Guidance Adviser (GA) will explain options for your child when they leave school such as their choice of further education, employment, training and other options including support from Social Care.
The ANA or GA will continue to support your child with their plans for the future, working closely with you, your child and their school to make sure that they make a smooth transition into adult life.
The need for support from Social Care in adult life (from the age of 19) will also be considered and if appropriate a referral will be made from the meeting for support from the Transitions Team.
When your child leaves school, the Statement of Special Educational Needs comes to an end but the planning for the future continues with the support of ANAs and GAs and, if appropriate, the Transitions Team.
Your child's views
In preparation for the review meeting, your child's school will help them to think about their hopes, dreams and ideas for the future and the help and support they will need. This is to make sure that their views are heard and taken into account at the meeting. Your son or daughter will also be invited to the meeting and encouraged to share their wishes for the future. If they have communication difficulties, you or a trusted support worker will be able to share their views at the meeting.
What you can do
You have a vital role to play in this process and will be invited to the review, receiving copies of the written reports prior to the meeting.
Before the review meeting you will be asked to give your views about your child’s progress and achievements and the best way of supporting them in the future. It may be helpful to gather together relevant information to take to the meeting so that you can provide details of your son or daughter's progress and achievements both in and outside school.
You are able to take an independent person to support you at the review. When the Transition Plan is drawn up, you will be asked if you are happy with and agree with the plan. If you are not happy with the plan, this will be recorded at the meeting.
