For disabled children our social care teams aim to:
- find out their care needs (an assessment)
- offer support, advice and information
- where needed, plan programmes of care for disabled children, young people and their families
- liaise with other agencies like health and education to make sure help is coordinated - including Early Support
The teams can provide information about:
- practical help at home, including equipment
- personal care
- respite care services
- playschemes
- social and leisure activities
- support groups
- transition from child to adult

Our working definition of disability is: "the needs of the disabled child or young person are beyond those of a non-disabled child of the same age and means they are likely to require lifelong support in the future from statutory services."
