If you want to see the information that the Council may hold about you, please complete the appropriate form from the two listed below.
Using these forms helps to ensure that you provide us with all the information we need to be able to progress your request.
The relevant form can be:
- completed on your computer as a Word document then printed out, signed and posted to us, or
- printed out, filled in by hand then posted to us.
If you are unable to print your form, please contact us letting us know which form you need, and we will post you a copy.

Please enclose photocopies of two items of identification with your form which together include your name, address, date of birth and signature (e.g. passport, driver's licence, birth certificate, utility bill). Please do not post your original documents to us.
Post your completed form and photocopies of ID to the address shown on the right.
How long will a request take?
Under Data Protection Act, we must respond to your request within 40 calendar days. This “clock” does not start until we have received all information required to process your request.
Will you be provided with everything we hold about you?
In the majority of cases, you will be provided with everything we hold. However in some cases, there may be information which is exempt from your request. If this is the case, we will write to you and explain the reasons for this.
