How do I make a request for information using the Data Protection Act 1998?

If you want to make a request under the Data Protection Act (a subject access request), you will need to do the following:

· Make the request in writing – either by post, fax or email asking for the information that we hold about you. 

In order to help us find the information that we hold, we will need the following information:

· your full name and date of birth
· your address and any previous addresses, if  they are relevant
· details of any services within CCC that you have had dealings with.

If you can also provide us with details of any reference numbers or names of the officers that you have had contact with, this will also help us to find all the information that we have.

On receiving your request, we will take steps to confirm your identity and will not release any information until we have done so.
