... what you need to know about CAF


The Common Assessment Framework (CAF) is about listening to you to find out what is working well in your life, and what more help you might need.  An action plan is then put together with you to make sure you get the right sort of help.

The CAF is there to help you but if you don’t want to take part in the CAF you don’t have to – it really is up to you.

If there are lots of people helping you, one of these people may be your ‘Lead Professional’.  This person will tell you what is happening, listen to any worries and support you. Your Lead Professional will also support the people who are helping you.

If you need to discuss CAF with someone other than you parent or carer you should contact your Lead Professional.  If you don’t have a Lead Professional  please contact the Locality Manager for where you live – the contact details on the right hand side.
