After an Assessment ...
... designing a set of services to meet your needs
If your care needs make you eligible to receive services, your assigned care manager will work with you to arrange the most efficient and cost effective way of meeting your needs - this is called Care Planning.
Ways of meeting your needs might include:
Home Care: if it does our leaflet "Home Care - What you can expect" gives more information.
Day Care: our leaflet "Day Care - What you can expect" gives more information.
Direct Payments: Following assessment of your needs, in most circumstances, we can offer you cash to arrange your own care services. These are called Direct Payments. More information on direct payments is available in our leaflet "Direct Payments". If you feel that this is your preferred option then we have a support agency that advise on how to use direct payments and how to make arrangements
Residential Care: If your level of need indicates that residential care is the most suitable option for you, our leaflet "Residential Care - What you can expect" has more information.
Our overall values and the standards to which we work are included in the leaflet "Social Care - what you can expect"
