Planning for the future is very important ...
The Learning Disability Partnership involves service users, carers, service providers and professionals from different organisations in all of its planning activities.
During 2006 we have been working up a wider Disability Strategy. This has general sections and specific sections that are just for people with learnng disability.
"Easy Read Summary of the Learning Disability Section from the Disability Strategy 2006" - A new Disability strategy has been written. This easy read document has the key areas for people with learning disability.
The following planning documents were produced between 2003 and 2006 but are still relevant:
"Integrated Quality Framework" - describes how the quality of services will be checked.
"Workforce Development Plan" - our plans to make sure we recruit, train and keep good quality staff.
"Person Centred Approach" - sets out how we will help people to get a plan to help them achieve the things they need.
"Definitions - terms used in Person Centred Planning and Health Action Plan" - this document includes definitions of Person Centred Planning - to help you understand what is involved.
"Employment Plan" - sets our our plans and aims to help people get paid or voluntary work.
"Day Services" - our plans to improve day services and opportunities for activities during the day.
"Housing and Support Plan" and the "Summary Housing Plan" (which is much shorter) - our plans for how housing and support services will be developed.
"Health" - our plans to help people access health support so they can keep healthy.
