Person Centred assessment of need
Care Management is the way Adult Social Care carries out assessments and the planning of care for service users who need help to live in the community. Sometimes an assessment looks at what general support and services (social care) a person might need. Sometimes a specialist assessment looks at a persons health needs. The assessment also looks at what support the family or carer might need. The person with a learning disability is helped to plan the type of services and support they need to be more independent and access their community. The planning of services should meet the needs which are identified during the assessment. Sometimes plans need to be changed as needs change.
Being Person centred is about:
- Listening to and learning about what people want from their lives
- Helping people to think about what they want now and in the future
Family, friends, professionals and services working together with the person to make this happen.
Reviews
We know that using a person centred approach is a continuous process but there are certain times when the LDP will check and review to see how things are going. These are called reviews. It provides a good opportunity to see what is working well and what needs to be changed. In day services we could use the time of the review to start to introduce a person centred approach.
