Coming out of Hospital and needing care at home?

Who to contact for help

Before old or frail people come home from hospital an assessment will be made of any personal care needs they might have so that they can be helped to manage at home. 

This assessment will be  done by social work staff in the Discharge Planning Team based at the hospital who will organise care as appropriate.

The details of how this process happens at each hospital will differ slightly.

If you are concerned this has not yet been done for someone you know, please speak to the Social Work staff based at the Hospital. 

Our page on Local Hospitals - Contact details will give you the telephone numbers for the hospitals in this area.  The switchboard at the hospital will put your telephone call through to the Discharge Planning or Transitional Care Team at that hospital.

If you would like some help whilst in hospital, the Patient Advice Liason Service (PALs) will provide confidential, on the spot, advice and support to help you sort out any concerns you may have about the NHS care you receive.
