Moving from Children's to Adults social care services
Planning for the future is important. To plan effectively for the future young people with a disability and their families need to know about:
- the choices and options available to young people and adults with a disability
- the process of moving from the support and services available to children and families to that available to adults
- who can get what support - the guidance and eligibility for services
- who to contact for information and advice.
When does the change take place?
In Cambridgeshire, children with a disability remain the responsibility of Children’s Social Care until their 19th birthday. At this time they need to transfer to adult services for the provision of:
- community support
- short breaks ( respite care, and Link carers)
- Direct Payments
- day and work opportunities
- or any other service originally funded by the Children’s Teams.
The planning for the move to support from adult services starts well before the 19th birthday. (see our Starting Early page)
