... planning for the life you want

Person centred planning is a way of planning for the life you want.  Many people with disability and their families think it is good way to think about what they want.  It can be used for both how things are now and for the future.

There are lots of ways that planning can happen but you are always at the centre of it. 

  • You are the most important person. 
  • It is not a one off meeting. 
  • It should carry on through your life.
  • It is a way of working together to make changes.
  • Some people use a circle of support to help them.

A circle of support is a group of people who know you well.  It might include family or friends.  The "circle" meet together on a regular basis to help you achieve your personal goals in life.

The circle acts as a community around you. It will help you to achieve what you want in life. You are listened to carefully at all times.

Time is taken to get to know you so you can build trust and communicate well
