Cambridgeshire's Transitions Team is made up of Social Workers who work with the Children’s Social Care Teams, Connexions and Education professionals to support young people with disabilities through the transition to adult services. Unlike Connexions who support all young people this team only work with young people and their families who are likely to require social care services as an adult.

Connexions staff work within schools to advise young people on educational and work opportunities available when they leave school.  Information about their role is on their website.

The Transitions Social Workers give advice and information to young people and their parents/carers as they near their 17th birthday or earlier if issues are complex. It is usual at this point to establish the best time for a Transitions (Community Social Care) Assessment to be completed by a member of the Transitions Team.

This can be arranged by a referral being made to the Transitions Team by the young person, a parent/carer or an involved professional, often as part of the Educational Review.

On referral, the Transitions Social Worker will offer a home visit to the young person and their family to provide information about adult social care services and the transition process. They will then carry out the transitions social care assessment with young people and their parents at the appropriate time and attend reviews as necessary. They will liaise with adult services to ensure they are aware of the young person’s future needs and that adult services make an application to their panel for services needed.

It is vital that adult social care are aware of the young person’s needs in time to plan and identify services that will meet their needs. The Transitions Social Worker will also assist an Adult Care Manager with any necessary funding applications within Social Care. This will try to ensure that the move to adult services is a smooth one.
