Social Care Assessments
Adult Social Care Teams provide care management and assessment of need for people with disabilities over 19 years of age. As we have limited funding we have to provide services to those in most need. To decide who we can provide services to we have rules called ‘eligibility criteria’. We check if the person meets the eligibility criteria, identify their needs and those of their carer where relevant. It is a person centred process and discussion takes place to make sure as much choice and control is given to the person as possible.
We look at things like:
- the safety of the person and others
- the physical health and disability of the person
- any mental health needs
- the person’s independence skills
- their involvement in work and social opportunities
- what support and contact with other people
To decide the level of need we score each area:
1 Low
2 Moderate
3 Substantial
4 Critical
Eligibility for services needs to be established and then a plan of what services can be made available is worked out. All of this is then taken to a funding panel for approval. Direct Payments or Individualised Budgets are available to people who would prefer to arrange their own services. More details are available on the relevant web pages. A support organisation is also available to offer support.
Who can get services from the Learning Disability Partnership (LDP)
The LDP provides both health and social care support to people with learning disabilities. We take an integrated and interdisciplinary approach to helping people with a learning disability who have social care or specialist health needs. We have locally based teams providing specialist support and advice for adults with learning disabilities.
The government arranges and funds health and social care in different ways. This means that there are different rules about who can get help.
We have guidelines to help us decide if we can help.
Health support is always free but sometimes there are charges for social care services. Assessments are free in both cases.
