An assessment identifies the social care needs of the individual. The Adult Social Care team then works with the young adult and their family to identify the support needed and how this might be provided. The type and amount of service may differ to that provided to the young person by Children’s Services.
It is not possible to include all of the options here but they include:
- community support
- short breaks ( respite care, and Adult Placement Scheme)
- Direct Payments
- Day opportunities, work activities and support into employment
- Supported living
There are charges made for services for adults, but these only aply following an assessmen of the persons ability to pay. Advice on Welfare Benefits which can be calaimed is available.
