For adults social work, we currently have openings in a range of areas including:

The Adult and Autism team covers the whole of Cambridgeshire supporting individuals from the ages of 18 years up to 75 years as well as their linked informal carers. They support those with Autism without a related learning disability, and others with eligible needs under the Care Act 2014 who do not sit with another statutory social care service. The team is based in St Ives, Cambridgeshire.

The Adult Early Help team provide advice and support to anyone over the age of 18, who is seeking support or social care. The team is made up of Social Workers, Occupational Therapists, Adult Support Coordinators and Welfare Benefit Advisors from a variety of backgrounds. They offer support to individuals and also to families and carers. The team is based in St Ives, Cambridgeshire.

The team support adults being discharged from a stay in hospital, by setting up care after they are discharged. The team refers people in need to a range of care services such as Reablement, long term care providers or other community care providers. The team is based in two locations one being Addenbrookes Hospital, Cambridge, the other in Hinchingbrooke Hospital, Huntingdon (with some cover in Peterborough Hospital).

The team provides integrated health and social care services to adults with a learning disability. Team members carry out individual assessments in a range of environments including the persons own home, the day service or in the community. The teams work alongside psychiatrists, psychologists, community nurses, occupational therapists, speech and language therapists, physiotherapists and health care assistants. The team are located across Cambridgeshire including Cambridge, Toft, Fenland and Huntingdon.

Mental Health services across Cambridgeshire are provided by the Cambridge and Peterborough Foundation Trust (CPFT). The teams cover a variety of areas, working alongside Consultant Psychiatrists, Psychologists, Psychiatric Nurses and Occupational Therapists with an aim of trying and help people recover or live with their illness in the most positive way possible. The team are located across Cambridgeshire including Cambridge, Huntingdon and Fenland.

The Older People’s team work with anyone over the age of 65, who are in need of care, support and safeguarding. The team aspires to enable people to make choices and take control to ultimately live healthy, fulfilled, socially engaged and independent lives. The team are located across Cambridgeshire with bases in Cambridge, Huntingdon, Fenland and East Cambridgeshire.

The team provides services to people aged 18 to 75 years, to support with a range of care needs with an aim to keep people living at home and within their communities for as long as possible. They do this by assessing and reviewing service users with a Physical Disability under the Care Act 2014. The team are based in St Ives, Cambridgeshire.

The Reablement team provide short term support tailored to their service users’ individual needs, to help them (re)learn the skills needed for daily living and reduce the amount of care and support they need. The team is made up of Social Workers, Occupational Therapists, Care Coordinators, Support Workers and Adult Support Coordinators. Their programmes can last from as little as a few days to a few weeks and are focused on helping service users achieve goals that they set for themselves to maintain their independence. The team are based across Cambridgeshire with bases in Cambridge, Ely, Huntingdon and Fenland.
