We work in the community to support older people, their families and carers. We provide a variety of services to support independent living at home which includes:

  • Free and confidential Information and Advice-maximising income, benefit entitlement, care provision, personal health and disability, housing, transport/Blue Badge and more.
  • Practical Services - handyperson service, installing grab rails and key safes, community warden services, hospital discharge support to settle back at home and admission avoidance support.
  • Social Opportunities - sharing Time home visits or telephone calls to reduce isolation and loneliness, meeting others at day services and friendship clubs.
  • Volunteering - lots of opportunities to get involved and help.
  • Hospital Discharge facilitation - help to go home and be supported whilst recovering (excluding social care packages)

Opening time notes

The calls are charged at local rates from landlines and mobiles (and will be free if used within your own free minutes plan). Our information officers will help with all your enquiries or can signpost you to someone who can. Between 9am and 4pm you will be greeted by one of the team, outside of these hours you will be automatically transferred to our National Helpline .

Eligibility

Not all services are available in every area, so you will need to ring us or look on our website to check where the service you require is delivered. Some services are available from age 60yrs, others from 65yrs, except Cambridgeshire Handyperson Service which is also available for any adult with a disability. If we do not provide the service where you live, we have the details of other providers, so please do call our information helpline and we will help to find the support you need.

Referral

Our information Helpline is open to everyone. Individuals can self-refer, or family on their behalf with consent. Professionals can refer directly. We facilitate hospital discharges for older people who are ready to go home without the need of a social care package but require some support (eg bed moves, grab rails, shopping, cleaning, keep in touch call). These referrals can only be made by hospital staff or staff or by our professional partners.
