Physical Disability Team
The Physical Disability Team provides services to people aged 18 to 75. We may support you with:
- personal care – things like washing and dressing
- supporting disabled parents
- social inclusion
We aim to keep people living at home and in their communities for as long as possible.
Reablement (Living independently)
We may provide a programme of reablement, short term support to help you look after yourself and live independently at home following an illness or operation. Support is tailored to your individual needs to help you (re-)learn the skills needed for daily living.
Residential and Nursing Home Care
If a programme of reablement is not suitable for you, we may assist you to find a residential or nursing home to meet your needs.
Support in the community
We may help you to access and arrange support in the community, including live-in care.
Equipment to aid independent living
The assistive/telecare technology team may provide equipment to help you to live more independently and where possible prevent you needing hospital or residential care.
Occupational therapy service
If you have a needs assessment this may be carried out by an occupational therapist (OT), who may also tell you about specialist equipment you can use at home to enable you to be more independent, or provide advice on adapting your home.
Blue Badge scheme
Parking concessions may be available to disabled people who qualify for a Blue Badge.
