There are two big issues driving forward the changes we need to make to sustain our high-quality public services:
- A demand for services that fit with our lives and do not box us into a ‘one size fits all’ approach.
- Pressure on services resulting from an increasingly ageing population, and people within our communities who have complex needs.
To meet these challenges we need to create a single community-based support system focused on our health and wellbeing. This requires a huge cultural and transformational change for services across the whole of local government and the wider public sector.
This gives adults who are eligible for social care support more choice over the services they receive and how these are delivered. It involves identifying and allocating a personal budget; an upfront sum of money to meet an individual’s needs.

Universal information and advice
An essential element in enabling people to make decisions about the support they need is accurate, accessible and timely information and advice. A new information and advice model for Cambridgeshire is being developed as part of work around the Care Act.
Reablement programmes provide short-term support following an illness or operation and enables people to continue to live independently by supporting them with the skills necessary for daily living. If at the end of their reablement programme a person requires on-going care and support they will be moved to self-directed support.

The shift towards prevention and early intervention is a key theme of Cambridgeshire’s vision for transforming adult social care through personalisation. Working and commissioning effectively with partners, particularly the Cambridgeshire and Peterborough Clinical Commissioning Group (CCG), district councils and other health providers, is at the heart of this work. Prevention and early intervention are key to Cambridgeshire’s new adult social care/social work model Transforming Lives and the Care Act.
This strategy provides an overarching framework outlining the vision, direction, priorities and long-term action plan for transforming adult social care in Cambridgeshire through personalisation.
