Learning Disability Partnership - community nurses

The community learning disability nurses help people with a learning disability have a healthy lifestyle and look after their body.

We can give you advice and support regarding long term conditions like:

Diabetes

We can also give you advice and support on keeping healthy including:

Diet

Having an annual health check and visiting your GP

Support at medical appointments including hospital, dentist, blood tests, scans and surgery

We can help make sure your reasonable adjustments are recorded and followed by all.

We also work with people whose behaviour can be challenging.

We can look at reasons for the behaviour like pain or infection.

We can monitor medication.
